HACC PYP Assessment and Service Planning

This interactive workshop will provide a refresher for experienced staff to reflect and build on their assessment and planning skills and consider ways to conduct more meaningful, person centred assessments with HACC PYP clients, and create care plans that add value for staff, clients and carers.  Staff will have an opportunity to review examples of their own work, seek feedback and discuss their experiences with colleagues.

 

This workshop will include discussion of how to:

•            Embed a strengths-based, trauma informed approach in your practice

•            Understand each person’s individual experiences, circumstances, needs and priorities through holistic, person centred conversation

•            Empower people to make informed decisions about their health and healthcare

•            Engage clients and carers to identify relevant and meaningful goals

•            Document relevant and useful care plans (including using appropriate language and strategies to reinforce a strengths-based approach and how to ensure key elements can be documented efficiently)

 

The workshop is aimed at experienced staff within HACC PYP services, both program staff and Living at Home Assessment staff.  The training will comprise of a full day in person workshop at the Alexandra Oval Community Centre, Ararat.  The event will be catered. 

The workshop will be facilitated by Kate Pascale and is available to all staff who are conducting assessments (LAHA and Service Specific) within HACC PYP Organisations operating in the Grampians region.
