The Commonwealth Home Support Programme (CHSP) delivers the entry-level tier of support and is designed to provide a relatively small amount of care and support to frail older people to help them to remain living at home and in their communities.
The Program Manual documents the operational requirements of CHSP. It also forms part of a funded organisations Grant Agreement.
Current Commonwealth Home Support Programme Manual and changes can be found here.
The Aged Care Roadmap has been developed by the Aged Care Sector Committee to outline what is required to realise a sustainable, consumer-led aged care market, where consumers have increased choice and control of what care and support they receive, as well as where, how and when they receive it.
myGovID has replaced the AUSkey method in order to prove who you are when using government online services such as the Data Exchange.
– Your Principal Authority will link your organisation in Relationship Authorisation Manager (RAM) and authorise your myGovID to transact on the organisation’s behalf.
– A Data Exchange Organisation Administrator must complete a User Access Request form to access the Data Exchange.
– Data Exchange Organisation Administrators can create other Data Exchange Organisation Administrators for their organisation.
The Data Exchange partners with funding agencies and organisations to support better use of data in gaining insights about client outcomes. Organisations participating in the partnership approach use the Data Exchange’s streamlined system to report outcomes data on their services. Outcomes data is collected using SCORE (Standard Client/Community Outcomes Reporting).
