The Commonwealth Home Support Programme (CHSP) delivers the entry-level tier of support and is designed to provide a relatively small amount of care and support to frail older people to help them to remain living at home and in their communities.
The Program Manual documents the operational requirements of CHSP. It also forms part of a funded organisations Grant Agreement.
The Good Practice Guide is intended to complement the CHSP Programme Manual and support the take up of wellness approaches in home care services.

My Aged Care is the entry point for older Australians to access aged care.

The Data Exchange represents the Commonwealth governments approach to grants program reporting. Funded organisations can provide data in three different ways:
  1. Direct entry through the web-based portal, which can be used like a free case management system
  2. System-to-system transfer
  3. Bulk uploads

AUSkey is a secure login that identifies a user participating in a government online services on behalf of a business.
Each funded organisation should have an AUSkey Administrator.
The key steps to access DEX are
  1. Obtain an AUSkey login. If you are not already registered for AUSkey, registration information is available on the Australian Business Register website.
  2. Complete the DEX User access request form and email the form to

myGovID and Relationship Authorisation Manager (RAM) will replace AUSkey at the end of March 2020.

The Data Exchange partners with funding agencies and organisations to support better use of data in gaining insights about client outcomes. Organisations participating in the partnership approach use the Data Exchange’s streamlined system to report outcomes data on their services. Outcomes data is collected using SCORE (Standard Client/Community Outcomes Reporting).
